Adult Protective Case Management

The Adult Protective Case Management program provides case management, representative payee, and trustee and guardianship services to County Adult Protective, Intensive Case Management (ICM), Veteran’s Administration and private pay clients. We also provide short-term assistance, advice and consultation to adults facing disability, and their caregivers. A state certified court evaluator for adult guardianships is also on staff.

ELIGIBILITY:

  • Monroe County Adult Protective clients age 18 or over in need of case management, representative payee or guardianship services.
  • Mental Health clients being served by a local mental health center and identified as needing representative payee services.
  • Services may be provided to other adults on a private fee-for-service basis. Also, individuals under 18 who receive Federal benefits and have a community case manager are eligible for fee-for-service representative payee service.

FEES: Services are paid either through contract with the referring agency, or paid from client funds. Payee service fees for Social Security and Veterans Administration benefits are set by Federal law. Guardianship and Trustee fees are set by New York State Law or Court Order. Other services may be arranged privately based on CFC’s fee schedule.

INTAKE PROCESS: Clients served under contract with Monroe County Department of Health Services Adult Protective or Coordinated Care Services, Inc Intensive Case Management Program must be referred by those agencies. We can assist in referring appropriate individuals to these sources.

Private pay clients may individually apply for service or be referred by a family member, legal counsel or social or governmental agency by calling CFC at (585) 232-1840, ext 376 (representative payee) or ext 138 (guardianship), and making the necessary arrangements for a court or SSA appointment. In some instances, the client may sign a power of attorney agreement.
