Early Intervention

The Early Intervention Program at Catholic Family Center has been in existence since 1998. Early Intervention is a program servicing families that have a child or children with special needs.

The program is contracted with the Monroe County Department of Health (MCDH). MCDH determines all eligibility.

Our target population is families that have a child from 0-3 years of age with a developmental disability. Disabilities may include:

  • Speech delays
  • Down Syndrome
  • Cerebral Palsy
  • Autism
  • Motor delays
  • Difficulty interacting with peers

We provide service coordination and social work services to families enrolled in the Early Intervention Program. These services are provided in the family home and by telephone.

Case Management services provided by our coordinators include referrals to community resources, referrals to other providers in the Early Intervention Program, and transitioning children into the preschool system.

In addition, social workers meet with families to discuss the impact that a child’s disability is having on the family unit. Social workers can make referrals to community resources based on family need.

ELIGIBILITY CRITERIA: The family must live in Monroe County and have a child between 0-3 years old with a developmental disability, as defined by the Monroe County Department of Health. The child must have a 25% delay in one area of development. These areas are: social-emotional, cognitive, communication, physical and adaptive. There are also some diagnoses that automatically make a child eligible for services. The most common diagnoses are prematurity, Down Syndrome, Cerebral Palsy and Apraxia.

INTAKE PROCESS: If you have conerns regarding your child's development, your first step is to call the Monroe County Department of Health at (585) 753-5437. The referral will be given to a initial service coorddinator who will contact the family and set up a multi-disciplinary evaluation for the child.
