As Monroe County’s designated information and referral line for older adult services, Eldersource answers more than 15,000 phone calls every year, yet our service is much more than a clearinghouse for information. Eldersource also provides guidance through home-based care management. In our care management role, we get to know you, your family and your unique eldercare situation. So whether you need the names of adult day care centers near you or help with a complex problem, Eldersource is your single source for eldercare guidance.
Whether you’re providing care for a frail parent or whether you’re trying to maintain your own independence, Eldersource can put you in contact with the right services and help you understand all the options.
Assessing What You Need
Making the right decision is hard. Our professional care managers, specially trained in geriatrics, can do an in-home assessment to evaluate individual circumstances. We evaluate the ability of older adults to perform daily living activities. We look at health history, home environment and even take into account social history. All these go into understanding what you need.
Eldersource Care Management Services, established in 1995, is a collaborative program of Catholic Family Center and LIFESPAN. It is our community’s largest provider of non-medical support services to older adults.
Eldersource is sponsored and supported by Monroe County and the United Way. Some Eldersource care management services are provided at a rate based upon your ability to pay. Once a care manager completely assesses your situation, we complete a care plan, link you to the appropriate services or benefits and provide ongoing guidance. We also monitor and even reassess when conditions change.
