PROGRAM OVERVIEW: PATHS (Professional Assessment and Treatment of Homebound Seniors) provides in-home assessment and counseling to homebound older adults experiencing symptoms of anxiety and/or depression which interfere with their daily activities. The expert counselors visit you in the privacy of your own home and work with you to reduce the impact of anxiety and/or depression.
ELIGIBILITY CRITERIA: You must be a Monroe County resident, be 60 years of age or older and experiencing anxiety and/or depression. You also must be residing in a community setting and unable to access traditional counseling services.
FEES: There are no fees, but donations are encouraged. Insurance will be accepted.
PROGRAM LOCATIONS:
- Offices at Catholic Family Center, 30 N. Clinton Avenue, Rochester, NY.
- Services are provided in-home.
