Rochester, NY… We are pleased to announce that Catholic Family Center is now accepting referrals to five Home and Community Based Services (HCBS) programs, starting September 15th. These services are exclusively available to HARP eligible members who are referred by Health Home Care Managers. We are ready to begin providing the services, but the first step of the process for your members starts with you!

These programs are designed to expand access to person-centered, trauma-informed support and training services provided by our compassionate and capable clinical staff and peers in settings that best suit your client's needs. These services are recovery-oriented, supporting care managers' integrated plans of care addressing physical and behavioral health needs through evidence-based and data-driven techniques that can build stability and promote hope, encouraging each person on their individual path to recovery.

  • Empowerment Services: Peer Supports – Connecting Clients with certified Chemical Dependency and Mental Health Peers to support their wellness and recovery through advocacy, engagement, training on self-help tools, and support through recovery, transitions in situation, and crisis.
  • Family Support and Training – Connecting clients and their family supports with support and training that facilitates healthcare and treatment professionals partnering with and including supportive family in treatment, recovery and/or rehabilitation.
  • Pre-Vocational Services – Preparing clients for employment through training and experiences that will build a foundation of strengths and soft skills that contribute to employability and stability in an integrated community setting.
  • Intensive Supportive Employment – Assisting clients in obtaining and keeping competitive employment, helping to identify reasonable accommodations that make it possible for clients who would otherwise struggle to participate in an integrated community setting.
  • Ongoing Supportive Employment – Follow-Along support, similar to Intensive Assisting clients in keeping competitive employment, allowing for ongoing follow up support that can build long term stability for clients.

Referral & Intake process

The referral process for HCBS is simple, and thanks to the assessment and eligibility criteria built into the HCBS model, it's easy to know which of your Managed Care clients are eligible. Once you've completed the HCBS Health Home Assessment, you can refer your clients to any of our HCBS programs through any of the following channels:

  • Please save the attached form and fill out on behalf of your client
  • Fax your referral to: 585-770-1116
  • OR scan and email the form to HCBS@cfcrochester.org

We're proud to offer these services, and we hope that you will reach out to your care managers and let them know how simple it is to include our HCBS services in their Plans of Care, and to refer clients into our programs.
