Home and Community Based Services are a group of programs designed to expand access to person-centered, trauma informed support and training services. These services are Recovery-oriented, supporting an integrated care plan that addresses physical and behavioral health needs through evidence-based and data-driven techniques that can build stability and promote hope, encouraging each person on their individual path to recovery.

This program is only accessible to Harp eligible members of a health home, with a Care manager to refer them to the programs we provide.
This program is paid for through Managed Medicaid already available to Health Home and HARP eligible clients.

School-based free counseling currently available at five schools for youth whose family environment, behavior and/or psychosocial development place them at higher risk for alcohol and substance abuse. Parents and/or siblings of these students may also be included in program depending on their involvement in these at risk behaviors.
The program currently serves children and adolescents in the Rochester City School District, the Hilton School District and Bishop Kearney.
Individual and family counseling, student group sessions, evidence based approach counseling, classroom presentations, connection to outside community supports.
Referred by school staff, students and parents within the identified school districts.
CD Counselor is contacted concerning referral, availability is checked to make initial referral assessment or placed on waiting list, and referral is admitted into program and/or outsourced to Community Resources.
Free of charge to eligible participants.

The Recovery Connection program provides intensive case management and outreach to clients with chemical dependency or co-occurring mental health disorders that have significant and high use of costly and intensive inpatient services. These individuals are often referred from inpatient treatment providers, detoxification centers, hospital emergency rooms, homeless shelters and other treatment centers. All of these clients have extensive basic needs for food, clothing and shelter, and ongoing income and insurance, are often still in active addiction. The goal in this program is assisting clients with their basic support needs, engaging clients into treatment for mental health and chemical dependency, developing abstinence and accomplishing their other individually defined goals.
The program provides extensive outreach and case management support to clients involved with the program. Care Coordinators assist with the location of safe and appropriate housing, facilitation in establishing or following through with medical or mental health services and supporting the client to address other key needs.
Clients must be residents of Monroe county, between the ages of 18 and 64, Medicaid eligible, have a history of hospitalization or inpatient treatments at least three times in 12 months or four times in 24 months and in need of case management.
