The Families Take Time program works with families with children -- ages birth through three years -- who have been diagnosed with or are at risk for developing non-organic Failure to Thrive Syndrome.
The program’s primary goal is to assist families in resolving the identified concerns that are impacting the child's weight and development.
Services included in the program are home-based counseling, specialized parenting skills training, education on infant care and nutrition, follow up on physicians' orders, weight checks and ongoing observation and assessment of parent and child.
All services are closely coordinated with the medical and community health care providers and any other community agencies involved.
The families must reside in Monroe County and have a child diagnosed with Failure to Thrive or are at risk of developing Failure to Thrive.
The program does not accept self-referrals. Referrals are made directly to the program social worker ((585) 546-7220 x4818) via pediatric care providers, visiting nurse services or Child Protective Services. Referrals can also be made through the Preventive Unit liaison at (585) 753-5821.
There is no cost to eligible families. The program is voluntary and families do not have to accept the service.
